The Quick Take
J&J beat on adjusted earnings, met on sales and nudged its full-year outlook higher. Adjusted EPS of $2.59 topped the roughly $2.50 consensus, though it fell 7.5% from $2.80 a year ago. Sales of $25.4B rose 5.9% and matched expectations of about $25.4B. Reported GAAP EPS collapsed to $0.09 because of a one-time charge tied to a proposed talc litigation resolution.

What Drove the Quarter
- Growth drivers: TREMFYA, DARZALEX, CARVYKTI, TECVAYLI, RYBREVANT/LAZCLUZE, ERLEADA, SPRAVATO and CAPLYTA in Innovative Medicine; electrophysiology and Shockwave in cardiovascular, contact lenses in vision and trauma in orthopaedics in MedTech.
- Headwinds: STELARA erosion cost about 690 basis points of growth in Innovative Medicine, with further pressure from SIMPONI, REMICADE, OPSUMIT and IMBRUVICA.
- Why EPS fell: Despite higher sales, adjusted earnings per share dropped, pointing to higher costs and investment, including recent deals such as Firefly Bio and the Sail Biomedicines collaboration.

What Management Said
CEO Joaquin Duato cited the “accelerating momentum of our focused portfolio” and said the company has a clearer line of sight to double-digit growth by the end of the decade.
Why the Numbers Matter
The portfolio is changing shape: newer oncology, immunology and cardiovascular products are growing fast enough to absorb the loss of Stelara and still deliver about 6% sales growth. But profit growth is lagging sales, with full-year adjusted EPS guided to rise only about 3%. The talc settlement charge removes a long-running legal overhang, which is a positive in the long run even though it makes the GAAP numbers look alarming this quarter.
Risks
- Stelara and other legacy franchises continue to erode.
- Adjusted EPS growth is far slower than sales growth; the guidance implies only modest earnings expansion.
- International sales grew just 1.5%, and MedTech 2.5%.
- Talc litigation resolution is proposed, not final.
- Acquisition and collaboration costs could keep pressuring margins.
Recommendation: HOLD
J&J is a defensive, cash-generative compounder with a strong pipeline, but this quarter shows earnings growth trailing revenue. A small guidance raise and a litigation clean-up are good news, not a catalyst. Existing holders can keep the position for stability and dividend income; new buyers should look for a better entry or clearer evidence that margins are turning up.
